Nature of Services. Midtown Biohack™, operated under the Morris Method™ framework, provides recovery optimization and human performance coaching services. These services may include but are not limited to: biometric assessments, recovery modality demonstrations (PEMF, red light therapy, hyperbaric oxygen, neuromuscular re-education), breathwork sessions, and Fulcrum Score™ evaluations. These are wellness and coaching services only — not medical care, diagnosis, or treatment of any kind.
Not Medical Advice. I understand that Dr. Chuck Morris, coaches, and staff at Midtown Biohack™ are not acting as my physician, therapist, or healthcare provider. Nothing shared during my demo experience constitutes medical advice. I am encouraged to consult my licensed physician before beginning any new wellness program.
Assumption of Risk. I voluntarily choose to participate in a demo experience at Midtown Biohack™. I acknowledge that participation in wellness and recovery activities involves inherent risks, including but not limited to physical exertion, sensory experiences, and exposure to recovery technologies. I assume full responsibility for any risk, injury, or damage I may sustain as a result of my participation.
Release of Liability. In consideration of being permitted to participate in this demo experience, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue Midtown Biohack™, Morris Method™, Nova Key™, Dr. Chuck Morris, their employees, coaches, agents, successors, and assigns from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained during my participation.
Health Disclosure. I confirm I am in generally good health and have no conditions that would prevent me from participating in a wellness demonstration. I agree to inform the coaching staff of any relevant medical conditions, injuries, or contraindications before my session.
Photography & Media. I consent to being photographed or recorded during my demo experience for Midtown Biohack™ marketing and educational purposes, unless I specifically request otherwise in writing.
Privacy. I consent to Midtown Biohack™ collecting and storing my personal information for the purpose of scheduling, communication, and follow-up related to my demo experience and potential membership. My information will not be sold to third parties.
Governing Law. This waiver shall be governed by the laws of the State of New York. If any provision is found to be unenforceable, the remaining provisions shall remain in full effect.
